Appointment Toolkit

Home » Appointment Toolkit

To make the most of your appointment consider the following "appointment toolkit".

  • Fill in the new patient intake form prior to your appointment so that the team already have a picture of your child and their journey so far.
  • A list of your concerns will assist us with developing joint goals and supporting your family in steps to achieve them.
  • Previous medical and healthcare reports, hospital discharge summaries, investigation results and a history of your child’s growth will assist with developing an appropriate pathway for your child.
  • Capture a record of what your child eats or drinks, how much and their feed times.
  • Any photos or videos that may give a clear picture of how they present at mealtimes can also help, as children do not always want to perform on their first clinic visit.

What to bring

We would love to see your child eating something that they enjoy as well as a food option that is more challenging. You may like to bring a few different choices with you just in case. Seeing your child’s usual eating and drinking utensils, cups and bottles are helpful in understanding their current skills, preferences and needs.

Preparing your child

For older children, it may be helpful to talk to them about the purpose of the appointment and what they might expect. We will talk about your child’s food preference and what they find more difficult about mealtimes and exploring new foods.
Please reassure your child that they won’t be expected to do anything that their not ready to do during the session.
